The following forms must be submitted to the UCSC Veterans Benefit Services Office

  • VA Intake eForm

Students can access the Veteran Benefit Services Enrollment form in MyUCSC by going to either the Graduate or Undergraduate Student eForm tile on their Student Homepage and selecting Veteran Benefit Enrollment from the menu.

Incoming students who are not yet eligible to enroll in classes can access the form in MyUCSC by going to the Application Status & Info tile on their Applicant Homepage and selecting the link in their Veteran Benefit Services Enrollment Admissions Message.

If you are a Visiting Student to UCSC or planning on attending another university as a Guest Student:

You must complete the UC Parent School Letter with your request and Certificate of Eligibility and/or Cal Vet Waiver Authorization Letter. Please see Page 2 for instructions on how to complete the Parent School Letter.
