Request Transcripts, Verifications, and Graduation Documents
Need proof of enrollment, degree completion, or your academic record? Learn how to request official documents like transcripts, enrollment or degree verifications, and certificates of completion. Most documents can be requested using your lived name, legal name, or both. You can update names and other personal details in the MyUCSC portal.
Enrollment Verifications
- Most lenders receive enrollment information electronically through the National Student Loan Data System (NSLDS).
- UC Santa Cruz sends enrollment, graduation, and withdrawal information to the NSLDS via the National Student Clearinghouse several times a quarter.
- If you receive a repayment notice, contact the Office of the Registrar to determine if further action is required.
Students can request an Enrollment Verification in person or by mail by using the Enrollment Verification Request form (PDF).
Third parties or outside agencies, including private loan and scholarship agencies, insurance companies, banks, employers, et cetera, may verify student enrollment directly through the National Student Clearinghouse.
Degree Verifications
- Students and third parties can obtain verification of degree completion through the National Student Clearinghouse.
- Transcripts are the official copies of a student’s academic history and the first place the degree appears on your record.
- Official Transcripts are available for order via Parchment, UCSC’s transcript vendor.
- Unofficial transcripts are available via MyUCSC for students who attended 1986 or later.
- Students who have completed their coursework but are still awaiting graduation, or need additional confirmation after graduation, may request a Certificate of Completion. See below.
Diplomas are mailed to the active permanent address two to three months after the quarter in which students graduate. Permanent addresses may be updated on MyUCSC.
Certificate of Completion
A Certificate of Completion shows that you will be receiving your degree or that you have received your degree. Certificates of Completion indicate the student’s name, the date the degree was or will be awarded, the type of degree (B.A., B.S., etc), and the student’s major, college, and honors, if any. Note that, in most cases a transcript is easier and faster to order, and provides better official documentation than a Certificate of Completion.
“Will be Awarded”
Students who have completed all graduation requirements, but are still attending a final quarter for personal edification, may be eligible for a Certificate of Completion with ‘will be awarded’ language. This certificate is only issued before the end of the term, after all graduation requirements have been actively completed, all required transcripts received, and after approval from the department.
“Was Awarded”
A Certificate of Completion is available to any student who has received a degree. If a student has applied for the most recent quarter, then a “was awarded” certificate can only be processed after the end of the term and after your degree has been awarded. To determine whether a degree has been awarded, view the unofficial transcript on the student portal.
To request a Certificate of Completion, fill out the Request for Certificate of Completion of Degree Requirements form, and send it to the Office of the Registrar by mail or email.
Letter of Reciprocity
A Letter of Reciprocity is an official document that can be provided for any student who has completed all of their lower-division general education requirements, and will include completion of university requirements (American History and Institutions and Entry Level Writing Requirement) if satisfied. A Letter of Reciprocity can also be provided for students who have completed their lower-division general education requirements. Students who have satisfied their general education requirements by Intersegmental General Education Transfer Curriculum (IGETC) are not eligible for a Letter of Reciprocity. Students with courses still in progress are also not eligible for a Letter of Reciprocity.
To request a Letter of Reciprocity:
- Log into MyUCSC (“Full Site Login” for mobile devices) and select Student Center.
- Select Undergraduate Student eForms
- Select Request Letter of Reciprocity
Other Verification Letters
On rare occasions, it may be necessary to get a verification letter from the Office of the Registrar that does not fall into any of the above categories. In such cases, please use this form to request such a letter. Placing a request with this form does not guarantee that a letter will be produced and letters requested through this process may take up to two weeks to be finalized.
