Q: How do I request a name change in AIS?[show]
A: The lived name will be used throughout AIS. Log into MyUCSC and select the Personal Information tile. Select Names from the left side menu. Follow the instructions on the page to add or update your lived name. If you do not see a Personal Information tile on your home page, select Self Service from the Navigator menu. Then select My Personal Information > Names.
Staff and faculty who are not current students may also change their own name in the campus directory.
Employees who require a legal name change can update their name directly on the UCPath portal. Changing your legal name in UCPath will not cause your name to be updated in AIS/MyUCSC. Faculty and staff can request a legal name change online after logging into MyUCSC. The form can be found in the Staff eForms tile in MyUCSC.
