Student Forms and Petitions
Looking for a form or petition? This page lists the most commonly used forms for enrollment, grades, graduation, transcripts, personal information updates, and more. Most forms can be submitted by email to registrar@ucsc.edu. Fees, if applicable, will be charged to your student account.
Tip: Do not send sensitive information (ex: Social Security Number) by unencrypted email. Please visit our office in person with your documents, or contact us to have a secured email thread started.
Enrollment and Grades
- Enrollment Verification Request – Shows proof of enrollment and expected graduation date.
- Retroactive Grade Change Request Form – Change P to C or NP to F to fulfill graduation requirements.
- Miscellaneous Letter Request Form – For situations not covered by other standard options
- Late Add by Permission – This has replaced the previous Add by Petition process. Visit our enrollment page to learn more about adding classes after the 3rd week of instruction.
- Credit by Petition – Get credit for classes outside normal circumstances, due by week 6.
Graduation
- Apply/Reapply for Graduation (Undergraduates) – Go here to read more about applying for graduation.
- Diploma Replacement Request – For lost or damaged diplomas.
- Certificate of Completion of Degree Requirements – Proof of graduation before diplomas are printed.
- Diploma Notarization Request Form – For circumstances where diploma notarization is required.
Transcripts
- Transcripts – See the information concerning Ordering Transcripts.
- Letter of Reciprocity – This documents completion of GE and University Requirements for students transferring to another UC.
Personal Information
- Request for Change of Name on University Records – Change legal name. If you have access to MyUCSC, read more here. Note required attachments.
- Request for Change of Social Security Number on University Records – Correct or add an SSN to your record.
- Request for Change of Date of Birth on University Records – Correct DOB on your record.
Special Programs
- Application for Cross Enrollment – Cross-enrollment in a course at UCSC for CSU and California Community College students.
- Domestic Exchange Program Application – Exchange program with the University of New Mexico (UNM)
- Intercampus Visitor Application Form – Enrollment at another UC campus as a visitor for one term.
- Simultaneous Enrollment with other UC Campuses – Cross-enrollment in another UC campus’ course not offered through UC Online.

- Petition for Undergraduate Individual Study / Independent Study Course – Work with an instructor on individualized studies
- Petition for Withdrawal/Leave of Absence from the University – Take time off from UCSC or leave permanently.
