Some forms are not online, but are available from the Office of the Registrar and sent by request directly to colleges and departments through interdepartmental mail.

Other forms are not available from Office of the Registrar but are available at the locations indicated below.

If you need to request forms or petitions from the Office of the Registrar, please send e-mail to the Office of the Registrar. Please include in your message:

  • Type of forms/petitions requested

  • Number of forms/petitions requested

  • Where the forms/petitions should be sent (campus mailstop)
